Pay:
$41/hr•$50/hr
Shift & Schedule:
Monday•Friday | 9:00 AM•5:00 PM | 35-40 Hours/Week Position Overview We are seeking an experienced Case Worker to provide social services support within a healthcare setting. The ideal candidate will assist patients and families with access to community resources, public assistance programs, and healthcare services while maintaining accurate case documentation and supporting individualized care plans. Responsibilities Assess client and patient needs to determine eligibility for available services and programs. Develop, implement, and monitor individualized service plans. Assist clients with public assistance, Medicaid, Medicare, and community resource programs. Provide counseling, support, and case management services. Coordinate services with healthcare providers, community agencies, and support organizations. Conduct investigations, follow-up activities, and eligibility reviews as required. Maintain accurate documentation and case records. Utilize electronic systems for case tracking, documentation, and care coordination. Ensure compliance with organizational policies and applicable regulations. Support patients and families in navigating healthcare and social service systems. Required Qualifications Bachelor's Degree from an accredited college or university. Minimum 3 years of case work, social services, or related experience. Strong communication, organizational, and documentation skills. Ability to work effectively in a structured healthcare environment. English proficiency (reading, writing, and speaking). Preferred Qualifications Experience with EPIC electronic health records. Experience with CarePort or similar care coordination platforms. Hospital-based case management or social services experience. Experience working with Medicaid, Medicare, public assistance, or social welfare programs. Strong counseling and interpersonal skills.
