Our mission is to promote the highest level of sustained recovery possible through individualized, effective, and affordable care. Job Summary Currently seeking a full-time Case Manager to assist clients with linkage to community resources. This position will require use of personal vehicle and will provide mileage reimbursement.
Requirements:
Associates Degree (or higher) 3 years of Behavioral Health Experience in lieu of degree (High school diploma required) Reliable transportationResponsibilities:
Conduct assessments of clients' needs, strengths, and challenges to develop individualized care plans. Coordinate and manage services, including medical care, housing, and employment, ensuring that clients have access to the appropriate resources. Provide ongoing support to clients, monitor their progress, and adjust care plans as necessary. Collaborate with healthcare providers, social workers, and other community organizations to ensure comprehensive care for clients. Maintain accurate case files and documentation, ensuring compliance with privacy and confidentiality regulations. Advocate for clients, ensuring that they are receiving appropriate services and support. Assist clients in navigating complex systems, such as healthcare, social services, or legal systems.
