Youth Case Manager Mayo Home for Youth Development - 3.0 Cincinnati, OH Job Details Full-time $18 - $20 an hour 3 hours ago Benefits Health insurance Dental insurance 401(k) Paid time off Vision insurance 401(k) matching Referral program Qualifications Driver's License Bachelor's degree Case management Driving Human Services Appointment scheduling Full Job Description Position Description The Case Manager (CM) will be part of the Mayo Home continuum of care providing counseling and coordination of services to youth residing in a Group Home setting. They interact with others in an ethical, culturally sensitive and respectful manner, being adaptive, flexible and responsive to the needs of the client and program. This person can perform a variety of tasks that include but are not limited to filing, answering phone calls, communicating with client case management teams, creating documentation and maintaining records. Excellent customer service externally/internally, strong problem solving skills, being organized, completing projects on time and understanding a sense of urgency is something they pride themselves in. The Case Manager is dedicated, has a positive energy, a team player, adaptable in different situations, and a quick learner. Essential Responsibilities Identify the needs of each youth and coordinate the necessary evaluations and assessments. Help clients overcome obstacles hindering the achievement of their therapeutic goals. Coach clients with social skills, interpersonal skills by role modeling Oversee client database is up to date online and offline Write Monthly Progress notes and Discharge Summaries in timely manner providing updates as needed to case management team Develop ISP and Discharge criteria with specific goals and objectives that are measurable with client Ensure compliance with applicable agency policies and in the electronic health record according to internal standards and external licensing and regulatory standards Obtain needed signatures from all parties involved for intake, discharge and monthly documentation Schedule required appointments and transport if needed Coordinate service learning projects Facilitate client based meetings Assist with monthly team meetings and document meeting minutes Collaborates with other social service agencies to ensure optimal treatment outcomes Manage aftercare with discharged clients and entering information into RTIS Maintain cleanliness of office Other duties as assigned Required Education & Experience 3 years of Behavioral Health Experience or Associates/Bachelor's degree/Master's degree in social work, psychology, human services or related field required Experience in Group Home setting preferred but not required Proficient in Microsoft Office and Google platform - nice to have Be a minimum of 21 years of age Must have a valid driver's license and good driving record A clean criminal background check The Mayo Home for Youth Development (MHYD) is a male only residence, offering at-risk youth a place they can call home.
