Full Job Description Case Manager:
Under general supervision, provide case management and/or specialized supportive and supplemental services to clients to aid them in improving their social, economic, cultural or educational status or to assist them in restoring their capacity for social functioning.

Responsibilities:
Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland. Provide clients with direct service, either in the office or in the field. Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients; provide service coordination. Assist clients in developing formal and informal community support. Provide advocacy on behalf of clients. Prepare and complete all required case recordings, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner. Attend clinical and administrative staff meetings, multi-disciplined team meetings as required. Review treatment plans and progress of cases with supervisor for continued support, advice, and approval. Perform other duties as necessary to accomplish objectives.
Requirements:
Combination of education and experience normally represented by a bachelor's degree and one to two years related professional experience. Must have competent oral, written and interpersonal communication skills. Must be well organized, detail oriented and have the ability to maintain confidentiality. Ability to work with diverse client population, community organizations as well as staff at all levels. Must be able to form good working and therapeutic relationships.
