Position Overview:
Family Promise of Greater Cleveland (FPGC), a respected non-profit with a 30-year history of aiding homeless families in Greater Cleveland, seeks a dedicated and dynamic Case Manager. This role is essential for providing comprehensive, trauma-informed case management services to families both in shelter and as they transition to community living. By embodying our mission to inspire and empower homeless families towards self-sufficiency, the Case Manager will be a central figure in our clients' journey from crisis to stability, ensuring seamless service delivery throughout their time with FPGC.
Principal Duties and Responsibilities:
Provide holistic case management for families, from shelter intake through to community integration employing trauma-informed and trauma-responsive practices. Develop individualized service plans focusing on housing stability, economic self-sufficiency, and family well-being utilizing evidence-supported approaches to promote effective outcomes Maintain regular contact with clients, including face-to-face meetings, to assess progress, update goals, and facilitate access to necessary resources. Coordinate comprehensive wraparound services to address the varied needs of families, ensuring seamless access to mental health support, educational programs, employment services, and other critical resources Collaborate with external agencies and internal departments to create a cohesive support network for each family, fostering long-term stability and self-sufficiency Document client interactions, progress, and service provision accurately in case files. Build and maintain positive relationships with landlords, property managers, and community service providers to support client housing and service needs. Participate in staff meetings, inter-agency collaborations, and professional development opportunities. Support the mission, vision, and values of FPGC in all interactions with clients and community partners. Perform additional duties as required, including support for Family Promise special events and programs.
Qualifications:
Bachelor's degree in social work, human services, or related field (will consider experience in lieu of formal education) Minimum one (1) year of experience in case management or a related field. Strong understanding of issues related to homelessness, poverty, and family services.
Skills and Knowledge:
Knowledge of and interest in human services, especially poverty, homelessness, and unemployment. Excellent interpersonal skills, including compassion and a sensitivity to social issues. Sound judgment and an ability to act quickly and professionally under pressure. Ability to work as a team player and support clients and professional staff.

Pay:
