Part-Time Case Manager - Youth Mental Health (MPAC Program) BF Empowerment Center Part-Time | Minimum 15 Hours Per Week Hourly Pay + Productivity Bonuses On-Site | After-School Program Setting Are You the Kind of Person Who Takes Initiative Without Being Asked? POSITION OVERVIEW
This position provides direct case management services to youth and families and facilitates a structured mental health therapeutic group within an after-school program setting. This is a minimum 15-hour per week part-time role , with opportunities for increased hours based on performance and program needs. Productivity-based bonuses reward initiative, engagement, and results.
This role requires someone who:
Takes ownership of their caseload Thinks creatively to engage youth Communicates professionally and consistently Follows through on commitments Is open to feedback and continuous learning
WHAT SUCCESS LOOKS LIKE
Youth are engaged and participating in therapeutic group sessions Families feel supported and connected to resources Documentation is accurate and submitted on time Cases are managed proactively, not reactively Productivity goals are met or exceeded The Case Manager contributes ideas for program growth and improvement
KEY RESPONSIBILITIES
Provide individualized case management services to youth and families Conduct assessments and develop service plans Connect families to appropriate community resources Facilitate engaging, structured mental health therapeutic group sessions Maintain timely and accurate documentation Communicate regularly with families and program staff Track progress toward goals and adjust plans as needed Meet productivity standards tied to bonus opportunities Participate in supervision, training, and professional development
WHO WE'RE LOOKING FOR
We are seeking someone who is: ✔ Self-motivated and able to work independently ✔ Dependable and consistent in attendance and follow-through ✔ Creative in engaging youth in group settings ✔ Open to learning and professional growth ✔ Organized and strong in time management ✔ Comfortable taking initiative without constant direction ✔ Professional in communication and documentation
MINIMUM QUALIFICATIONS 3
years of case management experience OR a Bachelor's degree i Reliable transportation and personal vehicle required Ability to work a minimum of 15 hours per week Ability to pass required background screenings
PREFERRED EXPERIENCE
Experience working with youth or school-based programs Group facilitation experience Knowledge of behavioral health systems and community resources Experience working in fast-paced or community-based environments Bachelor's degree in Social Work, Psychology, Human Services, or related field
