The primary focus of this role is to support clients throughout their recovery journey by coordinating care, connecting them to essential resources, and developing individualized treatment plans. The SUD Case Manager collaborates closely with clients, families, and healthcare providers to promote stability, wellness, and long-term recovery. Responsibilities Conduct comprehensive diagnostic evaluations to assess client needs and develop individualized treatment plans. Provide mental health counseling and support to clients dealing with behavioral health issues. Utilize knowledge of addiction medicine to assist clients in their recovery journey. Maintain accurate medical records, ensuring compliance with relevant regulations and standards. Implement psychotherapy techniques, including cognitive behavioral therapy, to facilitate client progress. Employ behavior management strategies to help clients develop coping mechanisms and improve their quality of life. Communicate effectively with clients, families, and interdisciplinary teams to coordinate care and resources. Monitor client progress and adjust treatment plans as necessary based on ongoing assessments. Qualifications Bachelor's degree in social work, psychology, or a related field; Master's degree preferred. Proven experience in mental health counseling or case management is highly desirable. Familiarity with addiction medicine principles is a plus. Strong understanding of medical terminology and behavioral health practices. Proficient in diagnostic evaluation techniques and treatment planning. Excellent communication skills, both verbal and written, with the ability to build rapport with clients.
