Job Overview The Veterans Service Coordinator supports residents in improving health outcomes and increasing self-sufficiency by connecting them with appropriate community resources and services. This position works collaboratively with the Manager of Resident Services to identify resident needs, provide support, and help residents access programs and resources that promote stability, independence, and overall well-being. Responsibilities Essential Job Function /
Responsibilities:
Provide service delivery for individuals with developmental disabilities and/or Veterans, ensuring compliance with all relevant regulations and standards. Provide general Service Coordination including intake, education, services available and application procedures and referral of residents to service providers in the general community.
These social services may include:
counseling, financial assistance, home health aides, homemakers, Meals-on-Wheels, preventive health screening, transportation and other needed services. Facilitate the formation of resident associations within the property if the residents are interested. The formation of these groups assists the residents in planning social events, organizing activities, and discussing daily life issues. Develop and implement individualized service plans that address the unique needs of each client. Maintain detailed notes regarding any assistance and services provided to the residents. Electronic and paper files must be kept in a secure location, accessible only to the staff (and management to the extent necessary with proper consent) and contain the following information: Maintain detailed notes regarding assistance and services provided to residents. Ensure electronic and paper files are kept in a secure location and are accessible only to authorized staff and management, as appropriate and with proper consent. Maintain resident files that may include intake forms, referrals to community agencies, service management plans, quarterly monitoring plans, follow-up notes, case status, and case resolution. Document information related to reports or concerns involving human or civil rights abuse, adult abuse, violations of personal property, or violations of dignity. Record results of monitoring, agency follow-up, meetings with residents and/or family members, and case disposition or termination. Collaborate with interdisciplinary teams to coordinate services and resources for clients. Maintain accurate documentation of client progress, services provided, and any changes in care needs. Foster positive relationships with clients, families, community partners, and stakeholders to enhance service delivery. Conduct regular assessments to evaluate program effectiveness and identify areas for improvement. Restabilize mental health by providing on-going training and resources to residents. Develop a Resource Directory that includes a listing of state and/or local service providers that residents can contact for assistance. Examples include services to families, children, individuals who are elderly, persons with disabilities, and emergency assistance. Frequently, state and local governments can also provide a listing of non-profit agencies they contract for services. Sponsor educational programming in conjunction with local companies and other non-profit organizations that can include subjects relating to literacy, computer technology, health care, agency support, life skills, referral sources, and others. Monitor the ongoing provision of services from community agencies and keep the case management plan current with the progress of the individual. Manage the provision of supportive services where appropriate and all other duties required by supervisor to support the Resident Services program. Serve as a liaison to community agencies, network with community providers, and seek out new services available to the residents. Assist the residents in building informal support networks with other residents, family and friends. Obtain and utilize the American Association of Service Coordinator's (AASC) Manual, utilizing and complying with the policies and forms that are contained within the manual. Comply with the AASC's Code of Ethics. When possible and appropriate, participate in HUD required training, Pathway HUB trainings and meetings and attend the AASC Annual Conference. Become familiar with housing industry laws and regulations concerning tenancy in the Community including, without limitation, the Fair Housing Act and 504 regulations (reasonable accommodation). Provide supervisor with a copy of the upcoming month's event calendar by the 25th of the month prior. Unless prohibited by law, HIPPA or professional ethical standards inform Community management of any resident incident or issue which adversely impacts the Community, or the safety/security of the Community. Have access to email services and access and respond to Community, St. Mary Development Corporation (SMDC), NCR and resident generated emails within 24 hours of receipt of next business day. Complete the AASC on-line monthly on-line report following the month being reported and provide necessary documentation to all required parties as requested. Other duties as assigned.
Qualifications:
A Veteran or Military Case Manager, Behavioral Health Case Manager, Social Service Case Manager, Community Health Worker certification, a bachelor's degree in social work, Psychology or Counseling is preferred, but not required.

Demonstrated working knowledge of supportive services and other resources in the area(s) served by the project. Minimum or 36 hours of training is required within 12 months of initial hire date and 12 hours annually thereafter. Training must cover an aging population and aging process, medication and substance abuse, mixed populations, the disabled population, Supportive Services needs of non-elderly people with disabilities, Supportive Services for the aging, Federal programs and requirements, State administered programs and requirements and entitlement programs, legal liability. Proficient experience and ability to use MS Word, Excel, and other software used by the organization. Must be able to pass pre-employment testing.
