JOB SUMMARY
The Outreach Case Manager provides needs assessment to incarcerated individuals and performs duties relating to case management including making jail visits, assisting clients in obtaining supportive services, making necessary referrals and providing training regarding independent living skills. This position also acts as an agency wide court liaison responsible for performing a variety of clerical tasks and admissions support.
ESSENTIAL FUNCTIONS
Essential functions are duties, which are essential or primary to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation. Assesses the needs of clients, assists in the development of treatment plans, and gathers information relevant to admissions. Acts as a direct link for referral sources to access treatment information and coordinate services with special orders from courts, law enforcement, and jails. Coordinates with courts, probation officers and other community resources to obtain supportive services that will aid the client in their personal recovery goals. Conducts educational and group counseling services in an effort to improve coping and daily living skills. Provides written documentation of client activity in order to maintain compliance with requirement standards and provides documentation for billing of services. Initiates all correspondence and paperwork related to assessments and client service reports (i.e., letters, progress notes, etc.) as needed. Provides input to treatment team regarding the developments of treatment plans and client progress. Meets with clinical staff and Program Director for review, input and consultation into client plan including open communication with Scioto County Jail, TCC Health, and Scioto County Outpatient Jail Linkage staff. Performs data entry functions including admissions intake and client information including record keeping and maintaining jail linkage data. Positively represents the agency through appropriate interaction, personal appearance, and attitude. Attend clinical staffing to assist clients with treatment needs, fulfilling legal obligations, and communication with referral sources. Provides oversight to community members and outposts about The Counseling Center and encourages a functional relationship between the two.
SECONDARY FUNCTIONS
Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center. Performs other duties assigned by the Program General Manager/Director. Assists in the report and evaluation of program goals and objectives.
COMPETENCIES
Demonstrated knowledge of alcoholism and chemical dependency Demonstrated knowledge/skill of clinical practice and performance, treatment planning, and counseling interventions Ability in the area of effective communication, interpersonal skills Ability to make decisions and to demonstrate sound judgment Ability to effectively manage self and tasks for maximum efficiency Ability to complete all required documentation in accordance with agency policy. Knowledge of federal and state laws, and public and private resources pertaining to counseling services.
MINIMUM QUALIFICATIONS, TRAINING AND EXPERIENCE
High school Diploma or equivalent, Behavioral Science Degree preferred. Prior counseling experience in outpatient or inpatient setting preferred, including experience working with chemically dependent people/families, experience with the 12-Step Recovery knowledge. Valid Driver's License and ability to maintain insurability preferred. Maintains applicable agency training
