Case Manager Substance Use Disorder (SUD) Outreach Full Time | Monday-Friday 8am-5pm Lutheran Social Services of Central Ohio is currently seeking a Case Manager in SUD Outreach for Faith Mission in Fairfield County. The ideal candidate will share
Our Mission :
Creating a better world by serving people in need. What will I do as the Case Manager in SUD Outreach with Lutheran Social Services? Engage individuals experiencing homelessness and substance use disorders through street outreach, community based contact, and relationship building using trauma informed and harm reduction approaches. Conduct intake interviews and ongoing assessments to identify substance use, behavioral health, housing, and psychosocial needs and determine program eligibility. Develop, implement, and regularly update individualized, recovery oriented service plans in collaboration with clients, focusing on stabilization, treatment engagement, and housing goals. Coordinate and facilitate referrals to substance use disorder treatment (including outpatient, residential, MAT), mental health services, medical care, housing programs, benefits, and other supportive services. Assist clients with obtaining documentation necessary for treatment, housing, employment, and benefits (e.g., IDs, birth certificates, income verification). Provide ongoing case management, follow up, and advocacy to support engagement, reduce barriers, and encourage sustained participation in services. Maintain accurate, timely, and complete documentation, case notes, and data entry in HMIS and other required systems in accordance with program, DBH, and agency requirements. Monitor and document direct client assistance expenditures and mileage to ensure compliance with program budgets and funding guidelines. Maintain current knowledge of community resources, treatment providers, and housing options, particularly those serving individuals with substance use disorders. Participate in team meetings, supervision, case conferencing, trainings, and collaborative work with community partners. Respond appropriately to crises, utilizing de-escalation techniques and established protocols, and coordinate higher level care when necessary. Perform other duties as assigned to support outreach and supportive services operations. Requirements for the Case Manager in SUD withLutheran Social Services:
Requires a minimum of an Associates Degree in Social Work or related field. Bachelor's degree preferred. CDCA (Chemical Dependency Counselor Assistant) credential or other relevant licensure, issued by the Ohio Chemical Dependency Professionals Board, required. 2-4 years of case management and/or crisis intervention experience. Experience with homeless or other special needs populations preferred. Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required.
