Job Description

Country Court Skilled Nursing & Rehabilitation Center is a locally and family-owned nursing facility in Mount Vernon, Ohio. We are looking to add a Case Manager to our team. We are looking for an individual that is a strong team player, has good communication skills, is caring and compassionate, and has a desire to make a difference in the lives of our residents. Previous experience as a Case Manager in long-term care is a positive but is not a requirement.   • Paid Orientation Job Description and Duties will include, but are not limited to; Coordinate care plans in collaboration with nursing, therapy, dietary, and physician teams Ensure resident needs, preferences, and psychosocial concerns are addressed in care planning Monitor changes in resident condition and communicate updates to appropriate team members Complete Admission paperwork with new admissions Coordinate safe and timely discharges, including: Home health services Durable medical equipment (DME) Transportation arrangements Provide discharge education to residents and families Serve as a primary point of contact for residents and families regarding concerns Assist with advance directives and code status discussions (in coordination with nursing/physician) Contribute to MDS assessments (especially Section Q and psychosocial areas)
