Job Description

The Counseling Center of Wayne and Holmes Counties is seeking a caring, professional Adult Therapeutic Behavioral Specialist / Case Manager for our Assisted Outpatient Treatment Program through Wayne County Probate Court to actively encourage and support the recovery of persons with severe and persistent mental illness; and assist impaired adults in achieving and maintaining successful community living. Now offering flexible schedule.
Duties:
Work collaboratively with Wayne County Probate Court, Assisted Outpatient Treatment Monitor, and the client's treatment team to coordinate client treatment. Collaborate with community partners, law enforcement, and inpatient hospitals to coordinate care. Submit required documentation to Wayne County Probate Court in a timely manner. Coordinates and reviews client evaluations to ensure accurate needs Develops with client, a coordinated treatment plan to address needs including empowerment, quality of life, access to care, housing, education, medical services, employment, and other areas identified by the patient Links clients to needed organizational and community-based services, advocates for services on their behalf, and ensures access to necessary services is achieved Monitors psychiatric symptoms/condition, compliance with treatment plan, and coordinates service delivery with providers both within and outside of the agency Facilitates access to transportation for appointments
