We provide compassionate trauma-informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve. We are currently seeking a dedicated and client-focused Case Manager for our Intensive Services team. If you have experience in case management, advocacy, and resource coordination and are passionate about helping individuals achieve recovery and independence, we encourage you to apply! About the Position As a Case Manager for Intensive Services, you will provide advocacy, linkage, case management, and independent living skills training to clients who require frequent contact to maintain community tenure. This position focuses on working primarily with Medicaid-eligible recipients and requires the ability to assess client needs, coordinate services, and deliver trauma-informed care in both office and community settings. Your role will include supporting clients in developing recovery plans, transitioning to lower levels of care, and accessing vital community resources. Additionally, you will provide intensive services for clients to assist in maintaining outpatient recovery and utilize individual assessments to determine the frequency of services. About the Program The Intensive Services program is designed to provide comprehensive support to individuals navigating recovery from mental health and substance use challenges. This program emphasizes a welcoming, recovery-focused environment that addresses the unique needs of clients who have experienced trauma. The program's goal is to empower clients to achieve stability and independence within their communities. Requirements Ability to obtain and maintain Case Management II (CM II) certification. Valid Oklahoma driver's license and ability to meet HOPE insurance requirements Skills Ability to assess client needs, organize services, and coordinate resources effectively. Strong interpersonal and advocacy skills, with experience working with individuals impacted by mental health and substance use disorders. Proficiency in clinical documentation and adherence to confidentiality standards. Capacity to work collaboratively within a team and provide services in diverse settings.
