Misdemeanor Diversion Program ("MDP") Case Manager will provide direct case management and coordination of needed services to clients.RESPONSIBILITIES MDP
Case Manager will focus on supporting the execution of the Diversion Hub's programs and mission. The Diversion Hub seeks to make lasting, transformative improvements in the lives of the clients that have been impacted by the criminal legal system. The responsibilities of the Case Manager include: Conducting regular intakes of clients, documenting progress notes, developing service plans, reporting critical incidences, collaborating with partner service navigators, and maintaining complete and comprehensive records for clients. Conducting client assessments to aid in the creation of individualized service plans that are focused on client safety, stability, and self-sufficiency, while maintaining a focus on assisting the client with getting out of the criminal legal system. Evaluating client risk and assessing the need for immediate intervention. Researching present and past criminal legal involvement, documenting progress notes, communicating with case managers on the criminal legal involvement of clients, responsible for tracking upcoming court-ordered appearances, and maintaining complete and comprehensive records for clients to assist them with compliance with the court system. Assisting with pre-trial and/or probation compliance. Maintaining court-ordered appearances for all clients so that clients can be notified by the Justice Navigator of the upcoming court appearances and in the case of an emergency, the Justice Navigator can provide the relevant documentation to the court excusing the clients' presence, while also re-setting the court date alongside the attorney. Serving as ongoing liaison with services navigators, judges, court clerks, attorneys, and all referring sources. Maintaining adequate communication channels ensuring a good relationship between referring agencies, staff, and clients. Submitting all required justice navigation reports timely for program activity which includes but is not limited to communicating program data, denoting activities, and changes of clients, as well as meeting all reporting requirements that ensure adequate operations. Providing exiting clients with satisfaction surveys when applicable. Assisting the client with developing self-sufficiency and stability by helping with overcoming barriers and meeting the expressed needs of the client. Facilitating client access to needed supports and resources such as but not limited to housing, employment, government benefits, food, ID assistance, clothing, referrals to mental health providers, referrals to physical health facilities, referrals to substance abuse services, vocational opportunities, drug tests, and court order classes. Always adhering to the agency code of ethics and professional standards including maintaining strict client confidentiality. Attending all staff development, training, and meetings as required. Any other duties within the scope, spirit, and purpose of the job and/or organization.EDUCATION AND EXPERIENCE 1
to 3+ years of experience in social work, communication, criminal justice, political science, or similar human service area.ADDITIONAL JOB REQUIREMENTS
Clearance of background check. Must have own transportation for site visits. Valid driver's license and insurance on the motor vehicle in accordance with state law and agency requirements.QUALIFICATIONS OF THE IDEAL CANDIDATE A

Passion and interest in reforming the criminal legal system. A hard-working, positive, mission-driven, and extroverted work style. A welcome attitude to constructive criticism of their work. Analytical skills, quantitative and qualitative.COMPUTER EQUIPMENT AND SOFTWARE REQUIREMENTS
Intermediate computer, mobile device, and internet skills with knowledge of software programs including but not limited to: Windows and Microsoft Office Products Cloud-based File Sharing such as Google Drive and Dropbox Adobe Products Client Databases SalesforcePHYSICAL DEMANDS
While performing the duties of this job, the employee must be able to sit over long periods of time and be able to talk, hear, sit, stand, and walk; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and stoop, kneel, crouch, or crawl climb stairs, lift and or move items of up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. Must have an excellent command of the English language and grammar, both verbal and written. Must be able to manually operate and use a computer.
