Job Description

Purpose The Housing Case Manager is dedicated to supporting individuals and families, connecting them with housing and other needed services. They provide the foundation for—and support—the culture of a community-based, client-centered, trauma-responsive organization. They will conduct assessments to determine client needs and eligibility for services, providing advocacy, education, and support to people in need. Responsibilities Client Advocacy The Housing Case Manager is responsible for representing clients and their families, empowering them through dynamic support, and amplifying their voices throughout the community to develop clients' resourcefulness, resilience, and self-sufficiency.
  • Determine client needs through formal assessments, interviews, and screening processes.
  • Determine which programs will be most beneficial; qualify and connect clients to appropriate resources.
  • Provide high-quality, trauma-informed case management services.
o Meet regularly with clients to build trust and confidence and to provide guidance and education. o Create dynamic and effective plans for client success. o Explore complex situations with clients, supporting them with problem solving methods and tools to provide immediate and beneficial assistance. o Provide resources and education to help clients learn and develop skills for self-sufficiency, such as budgeting and stress management. o Inform clients about new policies, such as county-level or state-level changes in housing laws so they can advocate on their own behalf.
For example:
- Explaining eviction moratoriums. - Communicating the impacts and requirements of new regulations and policies. Administration of Programs and Data Quality Management
  • Ensure prompt case notes, documentation, and data quality within Homeless Management Information System (HMIS), and other organizational data and case management systems.
  • Continually learn and stay up to date with new policies and program requirements.
  • Develop knowledge of resources and programs that will help clients and community partners.
  • Collaborate with Case Managers and other CCA staff to help clients access resources and information.
  • Develop relationships with community partners, landlords, property managers, community service agencies and others to ensure consistent access to support and services. Qualifications
  • Working knowledge of social services.
  • Ability to communicate professionally through email, telephone, and in-person to clients, throughout CCA, with community agencies throughout Clatsop County and beyond.
  • Proficient use of computer technology, including windows-based programs, database systems, and internet applications. Commitment to data quality improvement as an ongoing activity.
  • Demonstrated ability to work autonomously and as part of team through collaboration and engagement.
  • Valid driver's license, proof of insurance, ready access to reliable transportation, and insurance for business travel.
  • Preferred bachelor's degree in social sciences or related field and/or 2 years full-time equivalent human services work experience. Working Conditions and Physical Effort
  • The employee must be able to work well in an office environment. Due to the nature of the work, the employee may be required to deal with angry, confused, and/or frustrated clients.
  • The employee must frequently stand, sit, lift, and/or move up to 10 pounds, and occasionally lift and/or move up to 35 pounds.
  • Requires considerable walking (including climbing stairs).
