We are looking for compassionate and dedicated Case Manager to work in ShelterCare's Medical Respite (SMR) .What we offer:

This position works with individuals with high level chronic or acute medical needs and are experiencing homelessness who are referred to the 30-day Medical Respite program . During their 30-day, post-hospital recuperation, individuals can focus on their health needs while the Case Manager helps them create sustainable connections to community resources and medical providers. As an SMR Case Manager you will help participants identify health improvement goals, develop plans for achieving those goals, and support individuals as they navigate care systems. Documentation and data collection are critical to the success of this program. The Case Manager will also coach participants in effective management of their chronic health conditions and self-care. You will assist participants in understanding care plans and instructions, and facilitate communication and coordination between providers. See attachment for full position description. The SMR Case Manager is a non-exempt hourly position. The starting wage for this position is $23.68 - $24.50 per hour; depending on a number of factors. This position is considered essential and will be required to work holidays as scheduled . If it's a ShelterCare paid holiday, you will receive holiday pay plus pay for your time worked . The schedule for this position will include a working a weekend day (either Saturday or Sunday).QUALIFICATIONS
Required 3 years in healthcare or social services - or - Bachelor's degree in healthcare or social services; Awareness and understanding of the issues facing low-income people in the community and has an ability to work with consumers toward developing appropriate independence and increased self-sufficiency; Ability to drive agency vehicles Essential Skills Effective Communication; Negotiation and Collaboration; Scheduling and Time Management; Leadership; Risk Management; Critical Thinking and Problem-Solving; Use of technology for email spreadsheets, documents, etc. Preferred Prior electronic health records experience is strongly preferred; Experience working with individuals diagnosed with severe and persistent mental illness.ABOUT SHELTERCARE
