General Description:
This position plays an important role in supporting older adults and individuals with disabilities by assessing service needs, developing care plans, and coordinating services that promote health, safety, independence, and quality of life. Case Managers work directly with consumers and collaborate with community partners to connect individuals with resources and services that best meet their needs. Essential Duties Meet with consumers to assess service needs, daily living activities, and identify appropriate support options. Develop individualized care plans in accordance with program requirements and consumer needs. Coordinate services such as in-home care, residential services, transportation, medical supplies, and community resources. Monitor consumer needs and update care plans as circumstances change. Complete required assessments, documentation, and case records in Agency and State systems. Work closely with Agency staff, providers, healthcare partners, and community organizations to support consumers. Help consumers address environmental, medical, or social concerns and connect them with available local resources. Maintain confidentiality and comply with Agency policies, privacy regulations, and mandatory reporting requirements. ) Communicate directly with consumers whose primary language is not English. Interpret and explain forms, services, and program information in the identified language pair. Translate written materials as needed. Minimum Qualifications A qualified applicant will have a minimum of four (4) years of equivalent combination of education and/or experience which demonstrates the knowledge, skills and abilities required. The following qualifications meeting the minimum requirements will be considered: Bachelor's degree in social sciences or any related field Work or volunteer experience working directly with consumers in social service type settings. Other Requirements Driving for Agency Business. Positions that require driving must possess a valid driver's license and maintain an acceptable driving record. May require proof of personal automobile liability insurance in accordance with ORS Chapter 806 and use of a personal vehicle. Background Check. Problem-solving and decision-making skills in a professional service environment. Ability to build working relationships and provide respectful, person-centered customer service. Work Environment and Physical Demands Work is performed both in an office setting and in the community. Regular computer and telephone use. Frequent interaction with consumers, providers, and community partners. Occasional travel to homes, care settings, and community service locations. Frequent sitting, talking, hearing, and repetitive hand movement. Occasional standing, walking, reaching, bending, and lifting (up to 20 lbs). Interaction with diverse populations in office and community settings, including occasional exposure to contagious illness, challenging behavior, or domestic animals.
