Job Description
Housing Stabilization Case Manager The Housing Stabilization Case Manager supports individuals and families experiencing housing instability by helping them obtain and maintain safe, stable housing. This role works collaboratively with community partners to provide trauma?informed, coordinated care to vulnerable populations, including seniors, individuals with disabilities, and those at risk of homelessness. Housing Stabilization Case Manager Details:
Temp-to-Hire position $19.67 to $25.61 /hour Full Time work available First Shift (Monday through Friday from 08:00am to 05:00pm) Manage a caseload of households enrolled in housing stabilization or retention programs Screen and determine eligibility for housing?related assistance programs Assist participants with short? and long?term housing stability goals Maintain accurate case notes, documentation, and data entry in required systems Provide one?on?one participant support using trauma?informed practices Monitor participant progress to prevent a return to housing instability Refer participants to internal programs and external community resourcesHousing Stabilization Case Manager Qualifications:
At least 1 year of experience working with vulnerable or at?risk populations Strong interpersonal, written, and verbal communication skills Ability to work collaboratively with participants, coworkers, and partners Excellent organization, attention to detail, and time?management skills Proficiency with Microsoft Office and web?based applications Ability to adapt to changing priorities, schedules, and environments Valid Oregon driver's license with an insurable driving record Applicant may be subject to a background check Applicant may be subject to a Motor Vehicle Report Pre-employment drug screen (Excluding THC) is required as a condition of employment A conviction record will not necessarily prevent you from being employed.
