This role is for someone who brings steadiness, professionalism, and compassion to complex, fast-moving work. You will support adults and older adults with histories of chronic homelessness, very low income, and disabling conditions as they maintain housing stability and work toward personal goals. We are looking for someone who can build trust over time, maintain clear boundaries, and consistently show up for residents and the team. Permanent Supportive Housing provides long-term housing paired with voluntary, on-site case management, peer support, and community connection. Services are grounded in harm reduction, trauma-informed care, and participant choice. Residents are referred through Multnomah County Coordinated Access and supported in partnership with the Joint Office of Homeless Services (JOHS). What You'll Do Case Management & Resident Support Carry a caseload of approximately 15 residents Complete intakes, assessments, move-in coordination, and ongoing service planning Develop and update Individual Service Plans (ISPs) based on resident goals Hold consistent weekly or biweekly one-on-one meetings with participants Provide coaching, advocacy, crisis de-escalation, and housing stability support Connect residents to healthcare, behavioral health, benefits, and community resources Maintain timely, accurate documentation in HMIS and internal systems Team Collaboration & Reliability Participate consistently in scheduled team meetings, supervision, and trainings Coordinate closely with internal staff (Resident Services, THWs, interns, volunteers) Communicate proactively about caseload concerns, risks, or barriers Follow through on assigned tasks and shared responsibilities in a timely manner Contribute to a culture of mutual accountability and team support Community Engagement & Program Support Build and maintain relationships with external service providers and partners Participate in community-building activities and resident events Support outreach, data tracking, and occasional program communication efforts Our Approach to Service All services are grounded in: Trauma-Informed Care Participant-Centered Practice Strengths-Based Engagement Harm Reduction Philosophy We aim to support autonomy while maintaining healthy boundaries and consistent, professional service delivery. What We Are Looking For Core Competencies We are seeking someone who demonstrates: Consistent attendance and reliability in scheduled work hours and responsibilities Strong ability to maintain professional boundaries , even in emotionally complex situations Emotional regulation and steadiness in high-stress or unpredictable environments Ability to avoid over-identification and maintain participant-centered focus Strong follow-through, organization, and documentation habits Experience de-escalating crises and supporting participant autonomy Cultural humility and respectful engagement with diverse communities, including BIPOC, LGBTQ+, disabled, and low-income individuals Ability to work collaboratively within a team-based service model Preferred Experience 1+ year of case management, housing services, behavioral health, or similar direct service experience Experience in Permanent Supportive Housing, Coordinated Entry, or JOHS-funded programs Bachelor's degree in Social Work, Public Health, Human Services, or related field (or equivalent lived/professional experience) Familiarity with HMIS or similar case management systems Proficiency with Google Workspace Bilingual or multilingual skills strongly preferred Valid driver's license, insurance, and reliable transportation Compensation & Benefits Medical, Dental, and Vision Insurance 403(b) Retirement Plan and SEP-IRA Generous PTO and Sick Leave 12 Paid Holidays (prorated based on hours) Employer-paid Life Insurance Eligibility for Public Service Loan Forgiveness (PSLF) About Community for Positive Aging (CfPA) We are an inclusive, equity-centered organization supporting adults 55+ through care services, food access, community connection, and advocacy.
