Intesive Case Manager (Spanish speaking) The Lehigh Conference of Churches Overview Description 1. Must speak Spanish. 2. Outreach and identify client needs, including eligibility for services. 3. Set goals necessary to achieve stability, in cooperation with client. 4. Link with existing social services, community supports, housing opportunities/options, and all resources necessary for independence and stability. 5. Monitor service delivery through home visits, community appointments, and contact with other providers. 6. Advocate for clients with other service providers and coordinate services in place. 7. Maintain accurate records. 8. Other duties, as assigned.

Qualifications:

1. A minimum of a Bachelor's degree. 2. One year ICM or Human Services experience preferred. 3. Possess core competencies and experience necessary to work effectively with the mentally/physically ill, homeless, persons with chemical dependencies, the fiscally and materially challenged. 4. Valid driver's license, auto insurance, and car. 5. Ability to prioritize and plan daily schedule and meet deadlines. 6. Must speak Spanish.

$38,800.00 - $54,000.00 per year
