Job Responsibilities:
Work collaboratively with Bucks County Adult Probation and Parole, the correctional facility, and Family Service staff to receive referrals, coordinate assessment, review treatment options and housing resources to develop an individualized housing plan. Provide on-going support for overall health and well-being that includes health care, stable housing, improvements in family life and personal relationships, community connections, education and vocational development and employment. Assist clients in transition from correctional facility to temporary housing to permanent housing within 90 days. Develop assistance plan that includes rental assistance for up to 12 months including time in temporary housing. Make monthly payments to landlord that includes client contribution when applicable. Maintain complete and accurate case records, while ensuring confidentiality. Engage with and recruit landlords willing to give housing opportunities to individuals with criminal background. Represent BCOC at the Reentry Coalition for Bucks County, participate on committees and be an advocate for reentry housing efforts in Bucks County. Meet regularly with your supervisor, Adult Probation and Parole, and Family Service to review client progress as well as to report on client outcomes. Identify housing options, present available treatment and housing resources, assist with applications, and advocate on behalf of the client.Requirements:
Bachelor's degree preferred from four-year college or university; or two to three years related experience and/or training; or equivalent combination of education and experience. 2-3 years' experience working with reentry population. Valid driver's license, auto insurance, and use of reliable vehicle. Schedule may include occasional evening hours.Job Type:
