We pride ourselves on helping all of our clients to achieve more self-sufficient lives. Since 1957, we've assisted a wide array of populations throughout Southwestern Pennsylvania, focusing on those generally underserved by traditional services including senior citizens, veterans, individuals with physical and/or intellectual disabilities, individuals on the autism spectrum, and youths who have aged out of foster care. To further our mission, we are looking for a Housing Case Manager .
JOB SUMMARY
Working with the Rapid Re-Housing and the Permanent Supportive Housing Programs, the Housing Case Manager will provide oversight of housing plans for young adults experiencing homelessness. The position is responsible for coordinating comprehensive wrap‑around services to support clients in securing and maintaining stable housing. This role also ensures effective use of supportive services that promote long‑term housing stability.
ESSENTIAL FUNCTIONS
Provide housing support for young adults Ensure timely placement of young adults into appropriate housing in alignment with HUD requirements Conduct initial and monthly inspections in compliance with HUD standards Provide stable and desirable housing for young adults Meet with clients in the office and out in the community Develop and maintain positive relationships with clients Help to secure housing for program participants by visiting apartments throughout the Pittsburgh area and ensuring their cleanliness and safety Maintain and update necessary documentation for all young adults' files per DHS regulations Document casework activities and client interactions through clear, timely written case notes Assist young adults with moving into their apartments, including securing donations and purchasing essential items to furnish units Teach and model independent living skills Establish and maintain relationships with property managers and landlords in Allegheny County, and other service providers as needed to discharge job responsibilities Prepare all reports as required by funding agencies Other duties as assigned
JOB REQUIREMENTS AND EXPERIENCE
Bachelor's Degree in Social Services preferred Prior experience working with youth is required
