Job Description

Job Overview As a Homeless Assistance Program Case Manager, you will play a vital role in transforming lives by providing support and guidance to individuals experiencing homelessness. You will work directly with clients to assess their needs, develop personalized action plans, and connect them with essential resources. Your passion for helping others and your expertise in social work will empower individuals to achieve stability and independence. What you'll do Conduct comprehensive assessments to identify client needs and barriers to housing stability. Develop individualized service plans that incorporate workforce development, addiction counseling, and behavioral health support. Utilize motivational interviewing techniques to engage clients and foster a sense of hope and empowerment. Provide crisis management support, ensuring clients have access to immediate resources during emergencies. Collaborate with community organizations and service providers to facilitate access to Section 8 housing assistance and other essential services. Monitor client progress, adjusting action plans as needed to ensure successful outcomes. Maintain accurate case records and documentation in compliance with program standards. Advocate for clients' rights and needs within the community and relevant agencies. Requirements Experience in social support services is a plus. Personal experience with housing instability is a plus. Strong clerical, organizational and record keeping skills. Excellent crisis management skills with the ability to remain calm under pressure. Proficiency in motivational interviewing techniques to effectively engage clients. Exceptional communication skills, both verbal and written, with a focus on building rapport with diverse populations. Ability to work collaboratively within a team-oriented environment while managing individual caseloads.

$19.00 - $22.00 per hour
