Job Description
Job Summary We are seeking a dedicated and compassionate Assistant, Case Management to join our dynamic team. In this vital role, you will support individuals and families by coordinating services, providing crisis intervention, and fostering positive outcomes through effective case management. Your energetic approach and strong interpersonal skills will help empower clients to navigate complex social systems, improve their well-being, and achieve stability. Duties Assist case managers in developing and implementing individualized service plans for DD and ID individuals Conduct initial client assessments and provide crisis intervention when urgent situations arise Support clients with navigating placement in the Sponsor Residential Home Utilize motivational interviewing techniques to motivate clients toward positive behavioral change and goal achievement Collaborate with community partners and service providers to coordinate comprehensive support strategies Maintain accurate documentation of client interactions, service plans, and progress notes in compliance with DBHDS and the organizational policies and procedures. Supervise or mentor junior staff or volunteers involved in case management activities Requirements Proven experience with DD and ID individuals, crisis management, crisis intervention, or behavioral health settings Strong understanding of social work principles and motivational interviewing methods Experience supervising or mentoring team members or volunteers is preferred Familiarity with Sponsored Residential and healthcare programs, including, Medicare, and Medicade is advantageous Ability to handle sensitive situations with empathy, professionalism, and transparency Excellent communication skills to effectively engage clients from diverse backgrounds Valid driver's license and reliable transportation may be required for community outreach activities Join us in delivering impactful support that transforms lives!
