About Us Nurturing Care LLC proudly serves Medi-Cal members as part of California's Cal-AIM and DHCS initiatives. We are dedicated to improving health equity and access to whole-person, culturally responsive care for vulnerable populations, including individuals with mental illness, chronic health conditions, substance use disorders, and those experiencing homelessness. Position Summary We are seeking a committed and compassionate Housing Navigator/Lead Care Manager (LCM) to manage a caseload of 50 Medi-Cal members enrolled in Enhanced Care Management (ECM) and Community Supports (CS) Housing Services. This role focuses on monthly outreach, member engagement, housing navigation, and individualized care coordination to help members achieve stability and improved well-being.
Key Responsibilities:
Community Supports (CS) -
Housing Services:
Complete housing assessments and develop individualized housing support plans based on identified barriers and member goals. Set measurable short and long-term housing goals and establish strategies for achieving set goals. Assist members with locating and securing housing, including application completion and gathering required documentation. Provide Housing Tenancy and Sustaining Services, such as crisis management, conflict resolution, and ongoing landlord mediation. Coordinate with property managers, deliver housing deposits as needed, and support members with subsidy navigation and reasonable accommodation requests. Complete and maintain documentation for CS Assessments, Service Agreements, and Housing Care Plans in compliance with program standards. Support benefits advocacy and help members access housing subsidies and accommodations. Enhanced Care Management (ECM): Conduct consistent outreach using phone calls, telehealth, and in-person visits to engage assigned members. Complete Health Risk Assessments (HRAs) to evaluate medical, behavioral, and social needs. Develop and update individualized, person-centered care plans in collaboration with each member. Provide comprehensive care coordination, including: Scheduling and following up on medical and behavioral health appointments Facilitating communication between providers and support systems. Linking members to vital resources such as behavioral health, recovery programs, food assistance, transportation, and more Maintain monthly contact with each ECM member to ensure progress and make necessary care plan updates. Collaborate with an interdisciplinary team, including providers, housing partners, and community organizations. Meet productivity and reporting standards by documenting all activities in designated systems. Link members to community services, including behavioral health, substance use recovery, food, and transportation resources. Document services in electronic systems and meet required productivity and reporting standards.
Qualifications:
Experience and Skills:
Minimum 1-3 years in case management, housing services, or care coordination. Experience with vulnerable populations including those with mental illness, substance use disorders, chronic conditions, and/or experiencing homelessness . Knowledge of Section 8, Fair Housing, and social service systems. Strong crisis management, motivational interviewing, and communication skills. Technologically proficient and able to work independently.
Education:
Bachelor's degree in social work, human services, or a related field preferred.
Other Requirements:
Valid Driver's License, reliable transportation, and current auto insurance. Bilingual (Spanish) strongly preferred.
