This role focuses on ensuring the safety, stability, and well-being of youth by developing, implementing, and monitoring individualized service plans in collaboration with families, caregivers, and community partners. Case Managers serve as a key point of contact between DCS, service providers, and families, working to support positive outcomes through advocacy, resource connection, and consistent case monitoring. Responsibilities Collaborate with DCS Family Case Managers (FCMs) to understand case goals, court orders, and service standards expectations Develop and implement individualized service plans that align with DCS permanency goals Provide ongoing case management, including regular check-ins with youth, caregivers, parents, relatives and service providers Monitor client progress and adjust services as needed to ensure goal achievement Document all services, interactions, and progress notes in a timely and accurate manner Support caregivers with trauma-informed strategies, behavior management, and resource navigation Advocate for client needs in team meetings, case reviews, and court-related discussions Coordinate referrals to community resources such as counseling, education support, and life skills programs Ensure compliance with DCS service standards, policies, state regulations, and agency standards Participate in case staffing, Child and Family Team Meetings (CFTMs), and other required meetings Requirements Bachelor's or Master's Degree, or High School Diploma, GED, or Associate's Degree with four (4) or more years of full-time casework experience focused on child protection and care Valid Indiana Driver's License in good standing Vehicle Able to Pass all background checks Able to work evening hours If the Direct Worker possesses an Associate Degree, High School Diploma, or GED, they must also have: At least 4 years full time employment experience providing direct casework services to children & families that includes providing services to families that need assistance in the protection and care of their children
