ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING
Other duties may be assigned as necessary. Provides case management services to children and assists staff with compliance Facilitates placement of children Participates in development of assessment, service delivery, discharge, and aftercare planning. Reports any/all suspecting abuse or neglect to the Administrator on duty and they immediately report the information to txabusehotline.org Maintains regular contact with family and child and monitors psychological, mental, emotional, physical, and environmental well-being through phone contacts, email and initial home visits. Provides resource information and referrals to community organizations for therapeutic treatment such as behavior modification, stress management, and therapy to assist client in controlling disorders and other problems. Helps families develop a therapeutic milieu conducive to healthy child development. Provides crisis intervention as needed to prevent escalation of high-risk behavior, relapse, hospitalization, placement disruption, and other negative outcomes, while onsite. Provides "on call" services on a regular and equal basis or covers "on call" schedule - including providing awake, continuous coverage throughout the night to meet the particular needs of a child if needed; Consult with medical doctors and other specialists, and/or treatment team members concerning treatment plans and make recommendations as needed. Inputs and updates child and family information into the Casebook information system, in the interim will document on a case note. Ensures that medical and psychiatric appointments for diagnostic assessments are scheduled and completed within according to state regulations.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.
QUALIFICATIONS
The Case Manager shall possess experience in working with children and families. The ideal candidate will have excellent writing skills for case notes. The Case Manager should be able to develop trust with families in order to assist them with effective care for children in their home. EDUCATION, LICENSURE and/or
EXPERIENCE
The Case Manager shall possess a Bachelor's degree in a Human Services field with one year of related experience.
LANGUAGE SKILLS
Ability to read, analyze, and interpret professional journals and general business periodicals. Ability to write reports, presentations and business correspondence. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information and respond to questions from groups of clergy, parishioners, managers, clients, customers, and the general public.
MATHEMATICAL SKILLS
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
OTHER REQUIREMENTS
Valid Texas driver's license, appropriate insurance and reliable car. Must submit to and pass a pre-employment drug test. Must submit to pass random drug tests during employment. Must successfully pass all background screens as required by the state.
