NoABOUT THE AGENCY
As the only agency in San Mateo County with the sole purpose of serving those experiencing intimate partner abuse, CORA provides safety, support and healing. CORA s comprehensive services for clients include a 24-hour hotline, emergency shelter, legal services and counseling. In addition, CORA provides community education and prevention programming throughout the county. This Case Management position will be a vital role in CORA s Mental Health Department This full-time position will provide trauma-informed case management for families impacted by domestic violence This includes supporting both adults and children 0-17. This Case Manager will conduct therapeutic contact with clients and support intakes, screen participants, complete safety plans when needed, providing coping skills coaching to clients, develop case management plans and provide internal/external referrals. They will also schedule and coordinate family activities and outreach events. This position will be supervised by the Children s Program Coordinator and work closely with the clinical therapy team.PRIMARY RESPONSIBLITIES
Conduct case management intakes, screening, needs assessments and crisis case management for family survivors of intimate partner abuse (Domestic Violence). Meet with clients who are referred by CORA clinicians to create a case management plan. Provide immediate trauma-informed domestic violence support to clients identified by their clinicians as needed and appropriate, such as safety planning and coping skills coaching. Receive, coordinate, provide referrals, for the program to ensure the integration of a coordinated system-wide both internally at CORA and connecting families to external resources in the community. Proactively link clients to both internal and external resources based on their treatment needs, walk families through the referral process as needed (including accompaniment to services), and follow-up on the progress/status of the complete referral process. Provide consultation with clinicians who share your same clients, as needed. Document recommended case plan and activities in client record and other data management systems on a timely manner (within 3 business days). Serve as a liaison with the clinical therapy team, participants and clients, and a wide variety of community service providers. Provide administrative support, coordination and logistical preparation for all workshops and activities geared towards family engagement and psychoeducation. In partnership with mental health team, organize community outreach events and partnership meetings promoting CORA s mental health department and other programs. Maintain confidentiality and exercise professional judgment at all times in keeping with the responsibility carried personally and by the agency for the care and welfare of youth and families. Participate in meetings and trainings as assigned Other duties as assigned.REQUIRED QUALIFICATIONS - A
minimum of 2 years direct client service experience providing case management services to children, families, and at-risk populations. Proven ability to conduct intakes, screening, assess safety needs, develop case management plans, and provide follow-up and referrals for families and caregivers. Bilingual in English Spanish, both verbal and written, required. Ability to write case notes, reports and utilize Microsoft suite. Experience with utilizing a client data base system preferred. Ability to work with community collaborators. Knowledge of San Mateo County community resources preferred. Bachelor s degree from an accredited institution in social work psychology, counseling or related field preferred. AA degree and additional work experience can be offered in lieu of this requirement.WORK ENVIRONMENT/ PHYSICAL REQUIREMENTS
Successful results upon hire from a DOJ Live Scan fingerprint clearance are required Completion of the 40 Hour Training - required and will be provided to the successful applicant upon hire if not already completed within last 5 years. Driver s License and active insurance strongly preferredMANDATED REPORTER STATUS
