(LINC) is seeking a dedicated and trauma-informed Case Manager to provide comprehensive support services to justice-involved individuals participating in the Pathways to Community Program funded by the Governor's Crime Commission (GCC). This role focuses on crisis intervention, behavioral health support, housing stabilization, recovery services, workforce development, and reducing recidivism through proactive case management and community collaboration. This position is part of a grant-funded program. Employment is expected to continue through September 2027; however, continued employment is contingent upon grant funding and is not guaranteed beyond the life of the grant.
Key Responsibilities:
Conduct risk and needs assessments for individuals at high risk of crisis or violence Develop individualized service, crisis response, and safety plans Provide trauma-informed crisis intervention, de-escalation, and stabilization support Coordinate behavioral health, substance use treatment, housing, employment, and recovery services Support participants transitioning from incarceration, treatment, or crisis settings Collaborate with community providers, behavioral health teams, and peer support staff Maintain detailed documentation and assist with program data collection Utilize motivational interviewing and trauma-informed communication techniques Monitor participants for warning signs of relapse, instability, or escalating riskQualifications:
Bachelor's degree in Social Work or related human services field required Minimum of two (2) years of experience working with justice-involved individuals preferred Associate's degree with four (4) years of relevant experience may be considered Experience in case management, behavioral health, substance use recovery, and reentry services required Strong knowledge of trauma-informed care, Cognitive Behavioral Interventions (CBI), and community resourcesWhat We're Looking For:
