As an HCS (Home and Community-Based Services) Case Manager, you will support 30-45 individuals, helping them thrive in the community. You will lead a multidisciplinary team and oversee the development, coordination, implementation, and monitoring of services for your caseload. Shift:
Full Time, Monday to
Friday Hours:
09:00 AM - 6:00 PM (with a 1-hour lunch break)
Key Responsibilities:
Develop and write service plans for individuals on your caseload. Collaborate with individuals, their families, advocates, guardians, and external professionals to promote personal growth, safety, happiness, and fulfillment. Complete specialized documentation according to HCS standards within required deadlines. Utilize electronic health record software for most service-related documentation. Participate in routine case conferences to share suggestions and develop solutions for current issues. Engage in activities and events that support the lives of individuals. Monitor individuals in their residential and day activity settings to ensure their safety, security, comfort, and enjoyment.

Qualifications:
Valid Texas driver's license 2+ years of experience working with individuals with intellectual disabilities and/or related conditions Bachelor's degree preferred, but not required Case management experience, especially in an HCS program, is preferred Strong time management and organizational skills Initiative to identify and address challenges proactively Ability to remain calm and helpful when addressing concerns from family members Flexibility to assist in solving problems as they arise If you are passionate about making a difference in the lives of others, we would love to hear from you!
