Location:
Des Moines, IA (area for work).
Schedule:
35-40 hours/week, semi-flexible within 8 AM-5 PM M-F. Some on call time for evenings and weekends.
Reports To:
CEO About Betty's Project, Inc. Betty's Project, Inc. is a nonprofit organization dedicated to supporting vulnerable adults through compassionate, ethical guardianship and case management services. We advocate for dignity, autonomy, and access to essential resources for every client we serve. Position Summary We are seeking a highly motivated Case Manager to support the Betty's Project mission. This role involves direct client interaction with clients by visiting them in hospitals, nursing homes, and various community living situations to ensure needs are being met. The ideal candidate will be adaptable, empathetic, and capable of managing responsibilities with sound judgment. Patience is a needed skill as the systems the case manager works within can be challenging. Position involves travel within the Des Moines Metro area as well as to outlying areas on a bi-monthly or quarterly basis. Occasional travel to the main office in Coralville will also be required. Key Responsibilities Make routine visits to client per CEO direction. Conduct an assessment on the person, their providers and living environment. Attend nursing home care conferences and home and community-based services care planning meetings and assessments. Maintain accurate and confidential records by documenting time and assessment findings. Some data entry. Ongoing file maintenance. Obtain client information such as medical records, care plans, and vital records. Stay informed on changes in healthcare regulations and public benefit programs Facilitate the work of the CEO and Director. This includes visiting new clients in the hospital to enable video visits with the CEO and Director, presenting legal documents at banks and other financial institutions to obtain information, and accompanying the CEO or Director on various two person tasks in the Des Moines area. Qualifications Bachelor's degree in a human services-related field or equivalent or Associate Degree in Nursing (ADN) Must pass the National Certified Guardian exam within one year of hire (training and exam costs covered by organization) Strong interpersonal skills and emotional intelligence Ability to work with minimal supervision but humble enough to ask questions when needed Must be able to work independently and collaboratively Ability to manage multiple projects and adapt to evolving situations Previous experience in one or more of the following roles is a bonus: MCO case manager Group home supervisor, program manager or house lead Social work or care coordination in hospitals or nursing homes Bookkeeper, healthcare business office manager or similar with other personal or professional experience with the medical system, nursing homes and/or home and community-based services. Compensation & Benefits Competitive hourly rate based on experience $22.00 - $24.00 per hour. Semi-flexible scheduling within standard business hours (8 AM - 5 PM, Monday-Friday) 401(k) with employer matching Holidays and time off provided Additional Notes Candidates must demonstrate maturity, discretion, and a deep understanding of human behavior.
