Job Description

Case Manager-Residential Group Home Per Diem Healthcare Commons, Inc. Residential Group Home Case Manager (Per Diem) Healthcare Commons, Inc. is a not-for-profit, licensed community mental health agency that has served Salem County for over 70 years, as well as parts of Cumberland and Gloucester counties. Through multiple service locations, we provide comprehensive behavioral health and support services for children, adolescents, and adults, including outpatient counseling, residential services, crisis intervention, case management, and intensive family support programs. About the Role The Residential Group Home Case Manager (Per Diem) provides direct counseling, case management, and daily living support to clients residing in a group home setting. This role focuses on helping individuals increase independence, develop life skills, and successfully engage within the community. Key Responsibilities Provide individual counseling and supportive services to residents Monitor medications and ensure adherence to prescribed treatment plans Teach and reinforce Activities of Daily Living (ADLs) through individualized personal care programs Assist clients in building independent living and community integration skills Provide transportation for appointments, community activities, and other program needs Maintain accurate and timely documentation Support a safe, structured, and therapeutic residential environment Minimum Requirements •Must be available for evenings, weekends, holidays and overnight shifts Candidates must meet one of the following: High School Diploma and 4 years of experience in a group home setting OR 2 years of college and 2 years of experience in a group home setting OR Bachelor's degree in psychology, Social Work, or a related mental health field preferred Additional Requirement Valid driver's license in good standing required
