Facilitate the implementation of Individual Plan of Service. Perform comprehensive, ongoing psychosocial assessments. Participate in legal proceedings including petitioning and testifying in clinical emergencies. Provide community based services. Facilitate the coordination of consumer intakes, discharges, and transfers. Communicate with all service providers in Treatment Plan. Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker. Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
