NV Care Connection Reports to: Veteran Directed Care Project Director The Veteran Directed Care (VDC) Case Manager provides person-centered case management to Veterans enrolled in the VDC Program at the VA Southern Nevada Healthcare System (VASNHS). This remote position offers a high degree of flexibility and is well suited for an organized, self-directed professional who is committed to supporting Veterans in maintaining their safety and independence at home. The Case Manager establishes their own schedule and can complete most responsibilities from home. The ability to communicate with VA and State staff during standard business hours is required to support active cases. The position is up to 30 hours per week. Attendance at department and agency meetings at the JFSA office will be required. Other times may be required in the JFSA office as determined by the supervisor. The VDC Program allows eligible Veterans to self-direct their long-term care services using a flexible, VA-approved monthly budget. Within this model, the Case Manager acts as the primary liaison between the Veteran, the VA, and the Fiscal Management Services provider to support effective service coordination, spending plan alignment, and overall program compliance. For required home visits, agency vehicles are available for reservation and are prioritized for rural travel when possible.Key Responsibilities:
Manage an active caseload of VDC participants and provide consistent support through: o Monthly phone check-ins o Quarterly home visits o Additional contact as needed for changes in condition, emergencies, or care plan adjustments Conduct person-centered assessments and develop individualized service and spending plans in collaboration with each Veteran and/or their representative. Educate and support Veterans in exercising self-direction, including hiring, scheduling, and managing their own workers, which may include family members or friends when permitted by VA policy. Collaborate closely with the VA Southern Nevada Healthcare System point of contact and other VA staff to ensure coordination of care and compliance with VA guidelines. Partner with the Fiscal Management Services (FMS) provider (currently Acumen Fiscal Agent) to support payroll, budget tracking, and vendor payments. Monitor monthly budgets and spending trends; report any concerns or deviations to the VA. Maintain detailed, accurate, and timely case documentation in systems such as Welligent, Therap, or other program databases. Ensure compliance withHIPAA, VA
regulations, and agency confidentiality standards. Participate in required VA and NV Care Connection trainings, meetings, and quality assurance activities. Identify and report any health, safety, or risk concerns immediately to the supervisor or VA staff. Provide referrals to community-based resources through Nevada Care Connection, including additional long-term services, caregiver support, or benefits counseling. Support data collection and reporting requirements for monthly, quarterly, and annual reports. Reserve and utilize agency vehicles in accordance with established procedures for home visits; agency vehicles are prioritized for rural travel when available.Education and Experience:
Bachelor's degree in social work or a related field preferred; equivalent experience considered. Experience working with Veterans or military families strongly preferred. Experience supporting older adults or individuals with disabilities required. Experience providing case management or service coordination in a setting that requires independent work and minimal supervision strongly preferred. Demonstrated experience advocating on behalf of clients and assisting with navigation of community resources, public benefits, or support services required. Knowledge of Clark County community resources and Veteran service systems preferred. Experience with case management, service coordination, or self-directed care models (such as VDC, HCBS, or participant-directed programs) is desirable.Skills & Abilities:
Strong interpersonal, verbal, and written communication skills. Ability to build rapport with Veterans, caregivers, and interdisciplinary partners, and to work effectively with individuals from diverse backgrounds. Strong advocacy skills with the ability to problem-solve, remove barriers, and ensure clients receive needed services and supports. Demonstrated ability to work independently, prioritize competing tasks, and manage a flexible schedule with minimal oversight. Exceptional time management and documentation skills, with an emphasis on accuracy and follow-through. Proficient in Microsoft Word, Excel, Outlook; comfortable learning new systems. Ability to maintain confidentiality and adhere to HIPAA and agency privacy standards. Bilingual (English/Spanish) skills preferred but not required.Additional Requirements:
Valid Nevada driver's license and insurable driving record required. Willingness to travel throughout Clark County and surrounding rural counties. Access to an agency vehicle for home visits; use of personal vehicle may be required when agency vehicles are unavailable (mileage reimbursed per policy). Successful completion of background check, reference check, and drug screening.Working Conditions:
