Case Manager/Service Coordinator

The Brain Injury Association of New Hampshire is expanding and is looking for an individual to join a dynamic and enthusiastic team of professionals committed to providing quality case management services to individuals participating in the Acquired Brain Disorder (ABD), and Developmental Disabilities (DD) 1915(c) waiver programs. There is no direct care required. This is a 40-hour/week position with flexible hours, generous benefits package which includes vacations, holidays, and a 403B/Health/Dental package. The Brain Injury Association of NH is committed to a flexible work environment. •This caseload is located in Manchester/Hooksett as well as Rockingham County. You will see individuals in their homes and therefore MUST reside in this area. Travel is required (mileage reimbursement provided). This is a work from home position with travel to the Concord office mandatory at least for 90 days of training, and once monthly thereafter for staff meetings.•

Duties/Responsibilities :

Direct Services Work with assigned individuals/families who are living with chronic health conditions, brain injuries, and other disabilities on the community care waivers Meet with individuals and families in their homes to develop a plan of care Gathers pertinent information and records pertaining to the individual Reviews and organizes information gathered from various sources Consult with the individual/family to identify concerns and needed/requested services Conduct ongoing assessment and update planning document as needed Coordinate the delivery of services to meet identified goals and objectives Advocate for equal access to services to maximize individual functioning and opportunities for inclusion Consult with individual/family to ensure that services being received are appropriate, timely and meeting identified goals and objectives Consult with providers to ensure that the services provided are appropriate, timely and meet identified goals and objectives Guide individuals/family in resolving difficulties with agencies, access to services and/or service delivery Supports self-advocacy efforts Maintains information and records in accordance with established policies and procedures Indirect Services Serves as a consultant on brain injury issues for agencies and providers to ensure equal access to services and equitable treatment for all individuals Maintains confidentiality and standards of ethical practice Qualifications Bachelor's Degree in Counseling, Social Work, Psychology, Sociology or related field Understanding of disability issues Excellent communication skills necessary, both written and verbal Computer skills Ability to work independently Valid driver's license, reliable transportation, and adequate vehicle insurance coverage Must past background check, which includes criminal and
