Service Facilitator Agency Overview Triquestrian, LLC is a growing agency providing Equine Assisted Psychotherapy, Equine Assisted Learning, Psychotherapy, Skill Development, and Service Facilitation services to individuals and families. Our mission is to provide a variety of services to the community, creating opportunities for growth and learning. Position Summary The Service Facilitator assists clients participating in Comprehensive Community Services (CCS) through Dane County Department of Human Services by helping individuals identify and build a recovery team, coordinating services, and supporting recovery-focused treatment planning. The Service Facilitator gathers information related to clients' needs, strengths, goals, desired outcomes, and priorities as part of the assessment process and ensures that services are integrated, coordinated, and monitored to support clients in achieving the highest level of independent functioning. This position provides services in office-based settings as well as in the community. Full-time or Part-time position available Compensation:
$23.50-$25.50 per hour based on education and experience + benefits Essential Duties and Responsibilities Service Coordination and Planning Respond to referrals in a timely manner. Coordinate with intake staff and make initial contact with referred individuals. Build rapport and establish supportive working relationships with participants. Facilitate the establishment and implementation of Coordinated Service Teams. Develop individualized recovery plans within 30 days of the participant's CCS application. Conduct comprehensive assessments focused on client strengths, needs, goals, desired outcomes, and priorities. Develop measurable goals and review progress toward outcomes at minimum every six months. Conduct recovery plan reviews and updates as required. Client Support and Advocacy Utilize a strength-based, trauma-informed, recovery-oriented, and culturally responsive approach in all interactions. Coordinate mental health and/or AODA services with schools, medical providers, social service agencies, and other community partners. Locate, authorize, and coordinate psychosocial rehabilitation services, including mental health and substance use treatment services. Maintain active communication with providers and treatment team members to ensure continuity of care. Provide home visits, transportation to appointments, and accompaniment to treatment-related services when appropriate. Support clients and families in navigating systems and accessing community resources. Documentation and Compliance Complete service documentation within 48 hours of service delivery. Maintain confidential client records in compliance with program requirements and state regulations. Ensure all services and documentation meet DHS 36 and CCS standards. Participate in supervision, case consultation meetings, staff meetings, and trainings as required. Engage in continuing education and professional development opportunities. Qualifications Minimum Qualifications Bachelor's degree in Psychology, Social Work, Human Services, or a related field. Ability to pass a criminal background check in accordance with DHS 12 requirements. Valid driver's license, reliable transportation, and appropriate vehicle insurance coverage. Strong oral and written communication skills. Experience completing assessments, developing measurable recovery plans, and maintaining detailed documentation. Knowledge of mental health conditions, substance use disorders, and recovery-oriented care. Ability to work effectively and respectfully with children, adolescents, adults, families, and interdisciplinary teams. Preferred Qualifications Wisconsin Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or related licensure. Master's degree in Social Work, Counseling, Psychology, or a related field. Minimum of two years of experience providing case management or service facilitation to individuals with behavioral health challenges. CCS certification. Experience working within community mental health or human services systems. Work Environment and Expectations Position requires travel within the community, including home visits and meetings with providers and community partners. Ability to work independently while maintaining communication and collaboration with the clinical team. Flexibility to adapt to changing client and organizational needs.
