Job Summary We are seeking a dedicated and compassionate Community Case Manager to join our Sawali Disaster Case Management team. In this vital role, you will provide direct assistance to individuals and families impacted by natural or human-made disasters. This role involves assessing client needs, developing recovery plans, coordinating resources and services, and advocating on behalf of clients to ensure long-term recovery. The goal is to help clients regain stability and self-sufficiency. Responsibilities Conduct intake assessments to evaluate survivors' immediate and long-term recovery needs, including housing, finances, healthcare, transportation, and mental health support. Develop individualized disaster recovery plans with survivors and families. Serve as a primary point of contact for wildfire survivors navigating recovery systems and public assistance programs. Assist clients with applications, appeals, and documentation for recovery and assistance programs. Coordinate referrals to community resources, including food assistance, temporary housing, legal aid, employment services, childcare, and counseling programs. Provide trauma-informed, culturally sensitive support to Native Hawaiian, Filipino, and multicultural communities affected by the Lahaina fires. Advocate for survivor needs with government agencies, nonprofit organizations, landlords, and recovery partners. Maintain accurate case notes, recovery plans, and client records in compliance with confidentiality and program standards. Help survivors identify unmet needs and connect them to funding opportunities or rebuilding assistance. Participate in multidisciplinary case review meetings and disaster recovery planning sessions. Educate survivors about available recovery resources, rebuilding timelines, and community programs. Monitor client progress over time and update recovery goals as circumstances change. Requirements Bachelor's degree in Social Work, Human Services, Financial Services or Counseling; or extraordinary experience in customer service with track record proving ability of working with others in sensitive and challenging arenas. Minimum 1-3 years of experience in case management, disaster recovery, social services, crisis intervention, or community outreach is preferred. Strong understanding of trauma-informed care, cultural sensitivity, and client-centered service delivery is preferred. Ability to assess client needs, develop recovery plans, and coordinate multiple support services effectively. Excellent communication, interpersonal, and advocacy skills. Ability to work compassionately and professionally with individuals experiencing grief, loss, displacement, and emotional distress. Strong organizational skills with the ability to maintain accurate documentation, case notes, and confidential records. Proficiency with Microsoft Office, case management databases, and virtual communication platforms. Ability to manage a high caseload and work in fast-paced, emotionally demanding environments. Knowledge of local Maui community resources and multi-cultural values preferred. Ability to work flexible hours, including evenings or weekends, based on disaster recovery needs. Valid driver's license, reliable transportation, and ability to travel throughout Maui for outreach and home visits. Ability to pass background check and meet organizational compliance requirements. Bilingual skills (Ilokano, Tagalog, Spanish, or other community languages) are a plus. Demonstrated empathy, resilience, professionalism, and commitment to community recovery and equity. Ability to work collaboratively with nonprofits, government agencies, healthcare providers, and long-term recovery groups.
Pay:
