Job Description

The Consumer Directed Services (CDS) Specialist helps individuals with disabilities access and manage consumer-directed personal attendant services so they can meet their independent living goals. The Specialist provides case management, skills training, information and referral services, and maintains accurate documentation according to Missouri Department of Health and Senior Services regulations. This position is a mandated reporter in the State of Missouri. This role also includes advocacy, community outreach, and supporting the visibility of CDS services in the eight-county service region. Case Management Verify Medicaid eligibility for assigned consumers monthly. Contact newly assigned consumers within 2 business days. Provide one-on-one or group skills training to help consumers become effective employers of their attendants (EVV, record keeping, hiring, training, payroll responsibilities, etc.). Provide retraining when needed. Train new consumers on the caseload and enter documentation into CILS First within 3-5 business days. Complete yearly home visits and document in CILS First within 3-5 business days. Complete monthly phone contacts and document in CILS First within 3-5 business days. Ensure case notes clearly reflect the services being provided. Submit 45-day and 60-day letters when needed and complete a PCCP with 60-day letters. Complete PCCPs when a consumer needs a change in their plan of care or when the consumer is closing. Submit no-contact letters when unable to reach the consumer, ensuring all attempts are clearly documented with supporting details. Complete releases with consumers and scan them into the correct document folders. Complete in-house referrals when needed. Provide services according to all program guidelines. Ensure case notes reflect EVV compliance. Address compliance concerns with the consumer and complete a face-to-face EVV retraining if issues continue. Ensure community activities are correctly categorized in the database. Maintain clear, concise, and accurate documentation of all services in CILS First, including case notes, EVV record edits, PCCPs, contacts, and home visits. Identify additional needs and make referrals internally and externally (Vocational Rehabilitation, Medicare, Medicaid, Family Support Division, Social Security, etc.). Advocacy Support individuals with disabilities in accessing services to maintain or improve independence. Monitor for potential discrimination and assist with complaints as appropriate. Participate in outreach activities within the eight-county region and document them in the database. Serve on community task forces, committees, and councils as assigned.
PHYSICAL REQUIREMENTS
The physical demands described here are representative of those required to successfully perform the essential functions of this job. Sitting for prolonged periods of time, standing and walking on hard surfaces for short periods of time. Ability to travel independently to meetings or clients homes throughout the 8 county service area, as well as training throughout the state or country. Hearing and speaking ability to receive and provide verbal information, in person, on the telephone or via virtual meeting platforms. Vision sufficient to drive a vehicle, read, prepare, and proofread documents, read and enter information electronically, perform assigned duties. Requires frequent viewing of computer monitors, documents or printed information. Reaching, grasping and manipulating objects such as writing utensils, office items, paperwork and operating keyboards. Lifting and carrying light objects up to 5 pounds frequently and up to 20 pounds occasionally.
ADDITIONAL RESPONSIBILITIES
Perform office coverage as scheduled. Attend CDS meetings in person. Submit monthly expense reports on time and with accuracy, ensuring entries match CILS First, the calendar, and the in/out board, with minimal corrections required. Participate in staff meetings, CDS program planning, and internal training. Travel to conferences, workshops, and special events as required (including evenings, weekends, or out-of-state travel). Perform other duties as assigned to support the mission of the organization, including research and reference tasks.
