Coordinated Entry Case Manager Community Council - 3.0 Saint Peters, MO Job Details Full-time $16 - $18 an hour 1 day ago Benefits Professional development assistance Qualifications Professional networking Interpersonal skills Direct client contact Process improvement Bachelor's degree Full Job Description Community Council is currently seeking a dedicated Coordinated Entry Case Manager to support fragile families and individuals experiencing homelessness or housing instability in St. Charles, Lincoln, and Warren Counties. This role is vital to our mission, serving as a primary point of contact for clients seeking housing services and connecting them with the resources they need to thrive.
Position Details:
Location:
Hybrid (Community Council in St. Peters, MO & Remote)
Time Commitment:
40 hours per week (Initial 6-month contract, with the potential for extension based on mutual fit and budget availability)
What You Will Do:
Provide direct, housing-focused case management to clients contacting the Coordinated Entry Center via phone or walk-in. Conduct objective client assessments and ensure accurate, prompt data entry into the CaseWorthy database. Make timely housing referrals from the prioritization list and connect clients with appropriate community resources. Collaborate and build strong relationships with community stakeholders, including social services, healthcare, and education partners. Work closely with the Case Management team and Project Manager to resolve issues, identify unmet needs, and recommend procedural improvements.
Who We Are Looking For:
A professional with a Bachelor's degree in a related field OR at least two years of experience in human services. Someone with strong working knowledge of community resources available to low-income, homeless, or near-homeless individuals. A highly organized individual with proficient computer skills and a strong aptitude for accurate data entry. A team player who can balance the needs of clients, staff, and community partners while maintaining strict professional boundaries and client confidentiality.
