Case Manager/Fatherhood Coordinator Reports To:
Case Manager Supervisor Responsibilities:
Provides home visitation, case management, and health education to clients of The Family Tree Healthy Start program. The case manager will provide case management services that will include early childhood development, effective parenting skills, and appropriate child care. Services will be delivered in accordance with program time line policies. Responsibilities also include screening eligible families for risk factors, connecting enrolled families to a medical home, monitoring child health and referring families to community resources in order to meet their needs. The case manager will operate cooperatively as part of a multi-disciplinary team.
Qualifications:
Bachelor's degree in social services or related field. Experience in maternal and child health, working in culturally diverse settings, and knowledge of community resources preferred. Must have reliable transportation, valid driver's license, and appropriate vehicle insurance for fieldwork.
