Job Description
Overview We are seeking a dedicated and compassionate Guardian Representative to oversee assigned clients and services in a manner that supports the clients individual needs. The ideal candidate will possess a strong background in behavioral health, social work, or related fields, with a knowledge on adult welfare and protective services preferred. This role requires effective leadership skills to manage clients, coordinate resources, and ensure the delivery of high-quality services to those in need. Responsibilities Serve as a Lead Guardian Representative for assigned clients/wards. Make program required client visitations and notate all client interactions within the appropriate systems. Develop and implement client-based care that address the needs of clients, ensuring adherence to GFS best practices in crisis management and intervention. Collaborate with community partners, stakeholders, and agencies to enhance service delivery and promote client Support Plan and healthcare initiatives. Conduct assessments and evaluations to identify the needs of clients and assist with Client Support Teams to develop tailored action plans. Provide Court representation for assigned clients, as needed Organize dental and medical appointments; including travel Monitor program effectiveness through case management, data analysis and client assessments, making adjustments as necessary to improve outcomes. Advocate for client's rights and well-being within the community, ensuring compliance with relevant laws and regulations. Experience A degree in social work, psychology, or a related field is preferred. Proven experience in behavioral health or mental health is highly desirable. Familiarity with crisis management techniques and conflict resolution strategies. Knowledge of the court system, adult welfare policies and adult protective services Case management is essential. Experience working directly with clients in various settings is a plus. Strong interpersonal skills with the ability to communicate effectively both orally and electronically with diverse populations. Demonstrated leadership abilities with experience in effective teamwork preferred.
