Adecco Healthcare and Life Sciences is currently looking for a fluent bilingual Spanish/English Case Management Coordinator to support members through telephonic and face-to-face case management services. This role is primarily remote, with approximately 75% local travel to members' homes, assisted living facilities, and nursing homes throughout the region.

Requirements:

✔ Fluent in Spanish and English (reading, writing, and speaking) ✔ Must reside in one of the following ZIP codes: 33010, 33012, 33013, 33014, 33015, 33016, 33018, or 33027 ✔ Strongmunication,anization, and multitasking skills ✔ Tech-savvy andfortable working remotely ✔ Ability to manageplex caseloads and coordinate care effectively

Responsibilities:

Assess, plan, and coordinate member care Collaborate with providers, PCPs, andmunity resources Support medication reviews, prior authorizations, and condition management Conduct telephonic and in-person member visits Promote member wellness and positive health oues Monday-Friday Schedule Remote Training via Microsoft Teams (4-6 weeks) today to join a team dedicated to improving healthcare oues in yourmunity!

Pay Details:
