Job Description

  • Hybrid - Must Reside in Miami-Dade County, FL (West Hialeah Area)
Pay Rate:
$36.07/hr

  • fluent bilingual Spanish/English Case Management Coordinator
  • to support members through telephonic and face-to-face case management services. This role is primarily remote, with approximately
  • 75% local travel
  • to members' homes, assisted living facilities, and nursing homes throughout the region.
  • Requirements:
  • Fluent in Spanish and English (reading, writing, and speaking) Must reside in one of the following ZIP codes: 33010, 33012, 33013, 33014, 33015, 33016, 33018, or 33027 Strong communication, organization, and multitasking skills Tech-savvy and comfortable working remotely Ability to manage complex caseloads and coordinate care effectively
  • Responsibilities:
  • + Assess, plan, and coordinate member care + Collaborate with providers, PCPs, and community resources + Support medication reviews, prior authorizations, and condition management + Conduct telephonic and in-person member visits + Promote member wellness and positive health outcomes Monday-Friday Schedule Remote Training via Microsoft Teams (4-6 weeks) Apply today to join a team dedicated to improving healthcare outcomes in your community!
