  • Adecco Healthcare & LifeSciences is currently looking for a self-motivated, energetic, detail-oriented, highly organized, and tech-savvy Case Management Coordinator to join our growing Case Management team.
This position offers a Monday-Friday schedule with flexibility and autonomy while supporting members through telephonic and face-to-face case management services. Training will be conducted remotely through Microsoft Teams. This role requires approximately 75% travel throughout the Tampa region to visit members in their homes, assisted living facilities, and nursing homes.

  • Key Responsibilities
  • + Conduct telephonic and in-person member assessments + Coordinate and manage all case management activities for assigned members + Evaluate medical, behavioral, and social needs to support overall wellness + Manage a caseload including supportive and medically complex members + Develop proactive care plans and coordinate appropriate services and resources + Collaborate with PCPs, skilled providers, and healthcare teams + Assist with prior authorizations, medication reviews, condition management, and community resources + Maintain accurate documentation and case records + Support members in achieving short-term and long-term healthcare goals •Required Qualifications•+ Fluent in Spanish and English (reading, writing, and speaking) + Strong verbal and written communication skills + Ability to multitask, prioritize, and adapt in a fast-paced environment + Strong organizational and problem-solving skills + Comfortable using Microsoft Teams and other technology platforms + Ability to work independently while managing multiple responsibilities + Reliable transportation and willingness to travel throughout the Tampa area •Preferred Skills•+ Case management or healthcare coordination experience + Experience working with seniors or medically complex populations + Knowledge of community resources and healthcare services + Strong attention to detail and documentation accuracy •Additional Information•+ Remote/work-from-home opportunity + Monday-Friday schedule + Safety-sensitive position + Travel required throughout assigned region •
Pay Details:
  • $36.
