GLSS Case Managers work with older adults and people with disabilities to ensure they have access to services to allow them to live as independently as possible. As a Case Manager you will meet with older adults and people with disabilities to assess their needs, educate them about available services and assist them in securing the services they need.
Position Responsibilities :
Conduct in-home assessments of the consumer's overall situation to determine needs to create and implement person-centered care plans including identifying, coordinating, authorizing, securing and monitoring community based services on an ongoing basis. Modify and adjust care plans as the consumer's needs or situation changes.
Qualifications:
Bachelor's Degree in human services, social work, or related field preferred. Minimum of one (1) year experience in the human services field in lieu of Bachelor's Degree with the ability to obtain a waiver from Executive Office of Aging & Independence (AGE)
Proficient computer skills
Bilingual Spanish preferred
Expert time management with the ability to adjust to changing priorities
Strong desire to help others and advocate for those needing services and assistance
Excellent communication skills with the ability to establish positive and professional relationships with team members, providers, consumers, and their support system
Strong documentation skills with the ability to complete documentation that is timely, accurate, and comprehensive
The ability to appropriately handle confidential and personal information, meeting the Agency standards for HIPAA compliance and data security
Demonstrates a strong work ethic, dependability, and commitment to consistently meeting responsibilities and deadlines
Valid Drivers License and Reliable transportation with the ability to conduct home visits throughout the GLSS service area
Hours:
Full Time 40 hours per week (Monday-Friday)
Pay:
Starting pay range $22.50
24.50 an hour
