• Empowering Every Ability The Intake Case Manager is responsible for a variety of duties, this could include
• screening a member and determining if an assessment is appropriate, completing the assessment, pre-planning for services, reviewing cognitive/adaptive testing and determining if someone meets developmental delay or disability determination criteria.

Spanish bilingual preferred with a $1.00 increase in pay rate. The Intake Case Manager is also responsible for making referrals to community partners for people with long term care needs; this could include people with intellectual disabilities, who are elderly, blind, physically disabled or living with brain or spinal cord injuries or people with significant mental health needs. The Intake Case Manager is also responsible for the assessments for hospital admissions/discharges, PACE program, Skilled Nursing Facilities (SNF), and PASRR's.

Requirements:

• Bachelor's degree required; or five years of relevant experience in the field of Long-Term Services & Supports, which includes Developmental Disabilities.
• Driving is a requirement for this position.
• Case management and/or advocacy strongly preferred.
• Must be able to work some flexible hours and have own transportation and have access to a licensed and insured vehicle.

• 1 Full-time position at $27.05
• 29.75/hr. DOE/DOQ,
