Program Case Manager Humanmade San Francisco, CA Job Details Part-time From $32 an hour 5 hours ago Benefits Flexible schedule Qualifications Research Driver's License Bachelor's degree Organizational skills Social Work Full Job Description Overview of Role Humanmade is looking for a part-time Program Case Manager to join our team, serving Humanmade's workforce development clientele. This position will be up to 2 8-hour days per week, for a total of 16 hours weekly, and will occur in-person at our training facility in San Francisco. The Program Case Manager supports clients for the duration of their training experience, acting as their counselor and support to help ensure program success. This role is expected to work closely with and in tandem with the Employment Specialist for the greatest chance of success for our graduates. Once clients have been accepted to a program, the coordinator supports them through their transformative experience by assessing their well-being and helping to assist in the successful completion of the program by providing resource referrals. The case manager will conduct regular in-person meetings with each student for the duration of their training period. Upon graduation, the coordinator will continue to provide case management support for one year after training completion to promote continued success. The ideal candidate for this rewarding position should have an empathetic and outgoing personality and the desire to connect with a group of diverse candidates from traditionally underserved communities. The candidate should have a passion for helping individuals succeed and change their lives, providing support services to clientele through our workforce development and job readiness curriculum. About Us Humanmade is a non-profit community-based training, manufacturing, and prototyping studio on a mission to democratize access to the tools of innovation to individuals from all walks of life. Humanmade is the first of its kind, with an explicit mission around both training and inclusion in the maker movement. In addition to fostering a growing maker community, Humanmade strives to open doors and provide access to our studio by developing programs around workforce development and STEAM, among many more. Humanmade, with the support of key partnerships, helps to bridge the gap between simply learning a skill to building upon and applying those skills towards a sustainable career path.
Primary Responsibilities:
Duties will include, but are not limited to assessing, planning, implementing, monitoring and evaluating actions required to meet Humanmade's client's needs. In addition, networking and establishing relationships with employment partners. Facilitate multiple care aspects to support clients throughout their workforce training programs with referrals and resources for barrier mitigation Facilitate regular in-person and virtual meetings with clients and graduates to monitor goal achievement, individual learning plans, case management needs, resource referrals, and overall well-being Regular follow-up with clients post-training to assist in job acquisition and retention Help clients achieve success and autonomy. Engage in regular communications with clients, graduates, team members and community providers in a variety of formats, including email, virtual and in-person meetings, and phone calls Complete program documentation including case notes on client activities and meetings Develop referral relationships with partners and community-based organizations, and consult community providers for client resource/referrals Maintain relationships with all stakeholders
