POSITION SUMMARY
The Program Case Manager is expected to know and adhere to the Group Effort policies and procedures, and philosophies, and goals. Technical knowledge and competence in the Teaching Family Model training, consultation, and evaluation systems is essential to provide monthly consultative visits, advanced training, and monitor the FMP service plan. The PCM will be directly responsible for resolution of issues identified by the individual, DDA, MCO, family, natural supports and family model providers. Assures the administrative, support, supervisory, and Family Model Providers adhere to, and receive training and development in compliance with Group Effort and State of Tennessee regulations, Manage Care Organization's outcome standards, and other applicable laws /regulations.

): Manage caseload of 25-35 Direct participation in the development of the PCSP process and assist in the facilitation of Circle of Support meetings. Implement and participate in training programs and outcome directed activities, related to PCSP plan. Ensure that PCSP are being followed Act as a liaison for the person served by establishing community relationships. The PCM will be responsible for a minimum of one-two (1-2) "unannounced" supervisory visit a month to all family model sites. DDA-2 home visits, 1 Community. ECF- Monthly. CHOICES- Quarterly. As needed PCM will complete the medication processing and medication delivery. Monitoring community-based services to ensure quality of services and the integrity of freedom of choice by the individuals receiving services. Ensure that all reportable events are reported to the REM Coordinator Organize and maintain person supported Comprehensive, Home Binder Assist in the develop Risk Tools, LON, PSD, Available 24/7 to include face-to-face visit as needed when scheduled to work from home. Maintain communication with legal guardian, Family Model Providers, Support Coordinator, Care Coordinators and family members and maintain on-going documentation efforts of everyone served. Create service and fading plans Participate with meet and greets, 5-day meeting transition day, admission and discharge planning process. Obtain consents and medical releases and ensure all releases and consents are current. Ensure adaptive prosthetic and therapeutic devices are obtained as needed and maintained in proper working order. Participate in the accrediting process Participate on agency committees (HRC. REM) Ensure appropriate therapy are ordered (PT,OT,SLP,BA) Assist with TennCare redetermination as needed Monitor medical appointments and mediations 2 of 3
Transition Duties:
Assist with the care plan meeting 24-hour home follow-up home visit Assist with internal moves Communal Duties include Working a flexible schedule which will include occasional weekday or weekend overnight travel, serving on Teaching Family and/or Group Effort committees; this includes Participating in the "on-call" rotation. Available by phone (24/7)
Master Scheduling Oversight:
Assist with establishing and maintaining monthly consultation visits with the FMP.
Training:
Provide scheduled training to FMP on as needed basis.
Evaluations:
Responsible for completing annual evaluations for the
FMP. QUALIFICATIONS
This position requires 1+ year of case management or other related document and/or casefile management experience. SKILLS AND KNOWLEDGE REQUIRED
Education Bachelor's Degree (preferred) with at least one (1) year experience in a related field or High School Diploma/GED with two years (2) years' experience as a case manager, program coordinator or residential manager, Language skills Strong oral and written skills are required in this position including Ability to effectively present information to individuals, staff, and the general public including providing training on a variety of mental health topics Ability to respond to inquiries or complaints from customers, agencies, or members of the public Ability to write clear and concise reports and correspondence Ability to read, analyze and interpret professional articles, reports and documents Excellent skills in building and sustaining collaborative relationships Organizational and computer skills Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to compute percentages Reasoning Ability Ability to define problems, collect data, establish facts, and draw valid conclusions. Special Conditions and Compensation:
Pay Scale is $45,000.00 - $47,000.00. This is an exempt, full time 40 hour/week position.
