Program Screener 4.4 4.4 out of 5 stars 45 East 10th Street, Indianapolis, IN 46204 Hybrid work From $45,000 a year - Full-time HVAF of Indiana 5 reviews From $45,000 a year - Full-time Our Screening Team is a multi-member team that is tasked with interfacing with our clients at the point of a housing crisis. This Screener role requires front-line interventions to gather vital client information, triage client circumstances, and provide crisis interventions for veteran households. Principal Duties and Responsibilities
Works and collaborates with team members and community partner agencies to help identify, evaluate, and assist potential and current clients
Conducts initial phone screenings of referrals and determines eligibility for RRH programs
Tracks referrals and outcomes of client screenings
Completes Coordinated Entry System (CES) assessments for RRH clients
Maintains client records and documents services in HMIS, per agency standards
Works with potential clients to obtain identification documents and other needed documentation
Connects with all clients that present at or are referred to HVAF with a need of housing, including those not specific to Rapid Rehousing
Meets with walk-in veterans on an as-needed basis
Collaborates with team members to update, create, and/or execute new procedures
Coordinates with referral partners and establishes connections with new resources
Locates appropriate resources for clients and makes appropriate referrals
Attends community meetings to staff clients and act as agency liaison, specifically weekly Case Conferencing meetings
Actively engages with clients and actively maintains timely communication with clients, colleagues, supervisors, and community partners Education/Experience/Skills Required
Bachelor's degree in a related field is required; Social Work degree is preferred
Must have a valid Indiana driver's license and be eligible for HVAF's auto insurance at all times during employment per insurance carrier's requirements
Must be able to complete a background screening with an acceptable outcome
Ability to lift, move, and organize items up to 45 pounds
Willingness to be flexible and cooperative
Ability to work independently with strong problem-solving skills and effectively collaborate with other teams and departments within the agency
Acquired knowledge of all HVAF programs, understanding of programs, and ability to refer to HVAF programs
Demonstrated strong understanding and comprehension of programs and community resources
Demonstrated strong interpersonal skills and can effectively communicate verbally, in writing, and in presenting
Demonstrated strong time management skills and ability to prioritize according to urgency and deadlines
Knowledgeable of and able to provide referrals to other community resources
Knowledgeable of the factors associated with homelessness, substance abuse, and mental illness
Maintain a positive attitude and demeanor in all situations
Demonstrate proficiency in Microsoft Outlook, Word, and Excel
