Job Description

Job Summary We are seeking a dedicated and compassionate Qualified Professional (QP) to join our team. The Qualified Professional will provide support, coordination, documentation oversight, and person-centered services to individuals receiving mental health, substance use, and/or community-based support services. This position is hybrid, requiring two days per week in the office, with additional work completed in the community and/or remotely as needed. The ideal candidate will be organized, professional, dependable, and committed to helping individuals achieve stability, independence, and progress toward their treatment goals. Responsibilities The Qualified Professional will be responsible for completing and maintaining person-centered documentation, coordinating services, monitoring client progress, and ensuring services are delivered in accordance with agency, Medicaid, MCO, and state guidelines.
Primary responsibilities include:
Complete and update Person-Centered Plans, Crisis Plans, Treatment Team Meetings, and related documentation as required. Provide supervision and support to assigned staff, including CPSS and other direct care providers. Review service notes and documentation for accuracy, quality, and compliance. Coordinate care with providers, families, community agencies, MCOs, and other support systems. Assist with prior authorization paperwork and ensure documentation is submitted timely. Monitor client progress toward goals and update interventions as needed. Participate in team meetings, staffing, and clinical supervision as required. Maintain accurate records and ensure documentation is completed within agency deadlines. Provide support with referrals, linkages, and community resources. Follow all confidentiality, HIPAA, Medicaid, and agency policy requirements. Qualifications Applicants must meet the requirements of a Qualified Professional under North Carolina service definition standards.
Preferred qualifications include:
Bachelor's degree in a human services field such as Social Work, Psychology, Counseling, Sociology, Human Services, Criminal Justice, or a related field. Experience working with adults with mental health and/or substance use needs. Knowledge of Person-Centered Planning, Crisis Plans, service authorization processes, and Medicaid documentation requirements. Strong written and verbal communication skills. Ability to manage deadlines, organize documentation, and work independently. Experience supervising staff is preferred. Must be professional, dependable, and able to work well with a team. Person-Centered Planning in NC and Person-Centered Thinking Training Work Schedule This is a full-time hybrid position. The Qualified Professional is required to work in the office two days per week. Additional work may be completed remotely and/or in the community based on agency and client needs. Compensation Salary range is $43,000-$49,000 per year, based on experience and qualifications.
